What to Bring
A few things to have with you so your first visit goes smoothly.
Photo ID
A valid drivers license or other photo identification.
Insurance Card
Your dental insurance card, if you have coverage.
Medication List
A list of any medications or supplements you currently take.
Completed Forms
Any intake forms you filled out or printed ahead of time.
